Eligibility

Admission into the hospice program is based on medical eligibility as determined by a physician and a review by a hospice trained nurse. The primary criteria is that the patient must have a terminal diagnosis.

Each patient will have a complete assessment by trained and licensed physicians, nurses, social workers, chaplains, dieticians and CNA’s to determine an individualized plan of care.
